- Hospital discharge summary and birth records

- We schedule first visits within 3-5 days after hospital discharge

During Your Visit
We'll measure growth, conduct a physical exam, review development, and create a personalized care plan for your child's unique needs.
After Your Visit
Access health records through our patient portal, schedule follow-ups, request prescription refills, and message our team securely.
